Abstract

¸ñÀû: º» ¿¬±¸ÀÇ ¸ñÀûÀº ÇöÀçÀÇ Çѱ¹Çü ȯÀںзùü°è(Korean Patient Classification System-1, KPCS-1)¸¦ °£È£¿ä±¸µµ ¿¹ÃøÀ» À§ÇÑ À¯ÀǹÌÇÑ ¹®Ç×µé·Î ÃÖ¼ÒÈ­ÇÏ°í, °£È£¿ä±¸µµ ÃøÁ¤°á°ú¸¦ ÁßÁõµµ¿Í ÀÇÁ¸µµÀÇ 2°¡Áö ¿µ¿ªÀ¸·Î ³ª´©¾î ȯÀÚ¸¦ À¯ÇüÈ­ÇÒ ¼ö ÀÖ´ÂÁö¸¦ Ž»öÇÏ´Â °ÍÀÌ´Ù.

¹æ¹ý: º» ¿¬±¸ÀÇ ´ë»óÀº ÀÏ°³ »ó±ÞÁ¾ÇÕº´¿øÀÇ ¿Ü°ú º´µ¿ 2°³, ³»°ú º´µ¿ 2°³¿¡¼­ 14ÀÏÀÇ Á¶»ç ±â°£ µ¿¾È Àç¿øÇÑ È¯ÀÚ ÀüüÀÌ´Ù. 14ÀÏ µ¿¾È ÇØ´ç º´µ¿¿¡ ÀÔ¿øÇÑ Àüü ȯÀÚ¿¡ ´ëÇØ ¸ÅÀÏ 1ȸ¾¿ Çѱ¹Çü ȯÀںзùü°èÀ» Æò°¡ÇÏ¿© ÃÑ 411¸íÀÇ È¯ÀÚÀÇ ÀÚ·á 2,082°ÇÀ» ºÐ¼®ÇÏ¿´´Ù.

°á°ú: °¢ ¹®Ç׺°·Î ÃÑÁ¡±â¿©µµ Æò°¡¿Í Àü¹®°¡ °ËÅ並 ÇÏ¿© 50°³ ¹®Ç×À» 26°³ ¹®Ç×À¸·Î ´ÜÃàÇÏ¿´´Ù. ¿äÀκм®À» ÇÏ¿© À§»ý, ½ÄÀÌ, ¹è¼³, ¿îµ¿ ¿µ¿ªÀÌ ÀÇÁ¸µµ ¿äÀÎÀ¸·Î ¹­ÀÌ´Â °ÍÀ» È®ÀÎÇÏ¿´´Ù. ±ºÁýºÐ¼®À» ÅëÇØ ÁßÁõµµ¿Í ÀÇÁ¸µµ°¡ µÑ ´Ù ³·Àº ȯÀÚ(Æò±Õ Á¡¼ö: 7.68¡¾2.81/1.05¡¾1.33), ÁßÁõµµ ³ô°í ÀÇÁ¸µµ ³·Àº ȯÀÚ(Æò±Õ Á¡¼ö: 17.20¡¾4.15/1.94¡¾2.40), ÁßÁõµµ´Â Áß°£ÀÌ°í ÀÇÁ¸µµ°¡ ³ôÀº ȯÀÚ (Æò±Õ Á¡¼ö: 13.56¡¾5.30/9.66¡¾2.64)·Î À¯ÇüÈ­ÇÏ¿´´Ù. ¼¼ ±×·ìÀÇ °£È£¿ä±¸µµ ÃÑÁ¡Àº 8.73¡¾3.36, 19.14¡¾5.74, 23.24¡¾6.31 À̾ú°í Åë°èÀûÀ¸·Î À¯ÀÇÇÑ Â÷À̸¦ º¸¿´´Ù(F=1712.12, p<.001).

°á·Ð: Çѱ¹Çü ȯÀںзùü°èÀÇ ¹®Ç×´ÜÃà°ú ÁßÁõµµ¿Í ÀÇÁ¸µµ¿¡ µû¸¥ »õ·Î¿î ȯÀںзù±âÁØÀº ½Ç¹«¿¡¼­ÀÇ È°¿ë¼ºÀ» ³ôÀÌ°í ±¸Ã¼ÀûÀÎ Àη¹èÄ¡±âÁØ¿¡ À¯¿ëÇÒ °ÍÀÌ´Ù.

Purpose: The purpose of the study was to shorten theKorean Patient Classification System-1, KPCS-1 for predicting nursing care need level and to explore whether the patients can be clustered by their acuity and dependency.

Methods: The participants were inpatients in two surgical wards and two internal medicine wards at a teritory hospital during 14 days investigations. The KPCS-1 was evaluated once a day for all inpatients and 2,082 cases of data from a total of 411 patients were analyzed.

Results: The items were reducted from 50 items to 26 items by partial least squares analysis and expert review. Through factor analysis, it was confirmed that hygiene, diet, elimination, and exercise were categorized as dependence factors. Patients were clustered with low acuity/low dependency (average score: 7.68¡¾2.81/1.05¡¾1.33), high acuity/low dependency (average score: 17.20¡¾4.15/1.94¡¾2.40), medium acuity/high dependency (average score: 13.56¡¾5.30/9.66¡¾2.64) through cluster analysis. The total score of the three groups for their nursing care needs was 8.73¡¾3.36, 19.14¡¾5.74, and 23.24¡¾6.31 in order, and the results showed a statistically significant difference (F=1712.12, p<.001).
Conclusion: The shortening of the KPCS-1 and the new criteria for categorizing patients according to acuity and dependence will increase clinical utility and be useful for manpower assignment criteria in detail.
